The Third Person in the Exam Room

Imagine going to a dinner date with your spouse.
But there is a stranger sitting at the table.
Every time you try to order a meal, the waiter looks at the stranger.
The stranger checks a spreadsheet and says, "No, the steak is too expensive. Give them the side salad."
You would never tolerate that in a restaurant.
But you tolerate it in your doctor’s office every single year.
In every insurance-based medical visit, there is a third person in the room:
The Claims Adjuster.
You can’t see them. But they are running the meeting.
They decide which labs are "medically necessary" (usually the bare minimum).
They decide how much time the doctor can spend with you (7 minutes is the algorithm’s preference).
They decide if your treatment is approved or denied.
Your doctor might want to help you. They might know exactly what you need.
But if the accountant at the insurance company says "no," the answer is no.
It is hard to optimize your health when the person calling the shots is incentivized to spend as little money on you as possible.
The system isn't broken. It is working exactly as designed.
It maximizes profit by minimizing care.
Every denial is revenue for them. Every skipped test is a bonus.
The only way to get actual healthcare in 2026 is to fire the accountant.
Remove the third person from the room.
When you pay the provider directly, the incentives flip.
I work for you. Not the policy. Not the adjuster.
We run the labs you need, not the labs they allow.
We spend the time it takes to solve the problem, not the time the algorithm allotted.
If you want to be treated like a patient, not a liability, you have to leave the system.
The quality of care 10x’s immediately when the middleman leaves the room.
